Have your been re-housed by the local council?
Are you renting from a private landlord at the moment?
If the houses are being demolished have you been given notice by your present landlord when you need to be out of the property?
We need a few more details, if your new accomodation is with the Council, I would go ahead and move, it sounds like you are pleased with the offer.
Try and update your question with a few more details.
EDIT: I think your landlord is being terrible treating you like this, typical of a private landlord always money grabbing. This is obviously a legal issue I would contact Citizens Advice or Age Concern, they are there to help.

Can you not contact your council, and say that the disturbance is causing your distress, and you would like to move asap. It is not your fault that the council have not sorted this out, there obviously a property for you to go to.
Try asking your doctor to write a letter saying it is effecting your health.

isn’t it the council offering the “housing trade”???
if it is the council, then just phone them to confirm that you can still move.
if it is the owner of the house offering the trade, then ask that person if you can still go ahead with the move.
you must be clear on who it is offering the trade.
i know it is hard to think of moving..but it depends on who is offering the move..
if it is the council, make that phone call, and then go.
they may give you a few more incentives to help with the move.
also if it is the council, you do not owe the landlord anything to stay.
wait until you go into your new modern housing, with fresh new paint, clean new plumbing, modern heating, soundproofing, new electricial, no mould, modern appliances, etc.
you may even gain some health because of the standards new housing has with heating, electrical , and mould proofing.
